For most insurances, you will need a referral from another physician to see our providers. We cannot schedule an appointment without this referral. Our providers are specialists in their respective fields and as such, they review all prospective cases and decide if it is a case they can take on at that time or not. If our providers cannot take your case, we can recommend another specialist who would be a good fit for your needs. If our providers have reviewed your case and decide to take your case, then our office will contact you promptly to set up an appointment.
Once your appointment has been made, we will mail you a new patient paperwork packet. Fill this out prior to your appointment and be sure to bring that with you on the day of your appointment. Please bring your insurance cards, and any current imaging disks you have, i.e. MRI, X-ray, etc., with you to your consultation.
Before your appointment, plan accordingly so that you arrive 30 minutes before your appointment. When you come into our office, you will be checked in and invited to sit in our waiting room where we provide coffee, cookies and water. One of our medical assistants will bring you back to a room to take your vitals and ask questions regarding your health to relay to the doctor so he can be prepared for your visit together.
Once the doctor comes in, he will review your symptoms, your imaging, and together with you, come up with a specialized treatment plan.

For your first appointment, bring your completed New Patient Paperwork with you. Please bring your insurance cards, any information on prior medical treatment (surgery, physical therapy, etc.) and any current imaging disks you have, i.e. MRI, X-ray, etc., with you as well. It is best to arrive 30 minutes before your appointment, so we can put your information into our secure online database and give you time to ask any questions before seeing our providers.
Co-payments are due at time of visit. Please verify your co-payment with your health insurance carrier. Sometimes co-payments are higher for a specialist than they are for your Primary Care Provider.
Your medical file is handled with the utmost respect for your privacy. Our staff is bound by strict confidentiality requirements, such as HIPAA, regarding your medical records as a condition of their employment. We will not release the contents of your medical file without your consent.
You need to complete and sign a Medical Release form located in our Forms section. Your records will then be copied and you will be able to pick them up. Depending on the size of your medical chart, we can fax or mail the records to whomever you specify on your release form.
There will be a point of contact 24 hours a day for any concerns you may have. You will be provided with contact details following your treatment.
Please be sure to contact our office as soon as possible if you are unable to make your scheduled appointment with your physician.
Because our physicians are surgeons, there may be times when they need to be called out of the office for a surgery. We make every effort to give our patients as much advanced notice as we can in these situations. We realize your time is very valuable and we will work with your schedule to find another date and time that is convenient for you.
We take pride in our doctor-patient relationships and schedule plenty of time for you to get all your questions answered. If you need to get in touch with your surgeon in between appointments, please contact our office to leave a message or schedule an appointment.
