Making an Appointment with Our Physicians

For most insurances, you will need a referral from another physician to see our providers. We cannot schedule an appointment without this referral.

Our providers are specialists in their respective fields and as such, they review all prospective cases and decide if it is a case they can take on at that time or not.

If our providers cannot take your case, we can recommend another specialist who would be a good fit for your needs.

If our providers have reviewed your case and decide to take your case, then our office will contact you promptly to set up an appointment.

What to Know Before Your First Appointment

Once your appointment has been made, we will mail you a new patient paperwork packet.

Fill this out prior to your appointment and be sure to bring that with you on the day of your appointment.

Please bring your insurance cards, and any current imaging disks you have, i.e. MRI, X-ray, etc., with you to your consultation.

What to Expect on Your First Visit

Before your appointment, plan accordingly so that you arrive 30 minutes before your appointment.

When you come into our office, you will be checked in and invited to sit in our waiting room where we provide coffee, cookies and water.

One of our medical assistants will bring you back to a room to take your vitals and ask questions regarding your health to relay to the doctor so he can be prepared for your visit together.

Once the doctor comes in, he will review your symptoms, your imaging, and together with you, come up with a specialized treatment plan.
